Provincial: Ontario Disability Support Program (ODSP), Family Responsibility Office (FRO), Birth Certificates/Birth Registration, Driver’s Licenses, OHIP, OSAP, London Hydro, Ontario Works (OW), Landlord-Tenant guidance (depending on the situation). Contact your MPP Terence Kernaghan for assistance.
Municipal (City of London): Roads & Road Safety, By-laws, Parking, Noise Violations, London Transit, London Housing, Building Permits, Marriage Licenses. Contact your City Councillor for assistance.
How to Make an Appointment with Our Office
You can easily set up an appointment by either phone (519.663.9777) or email (Peter.Fragiskatos@parl.gc.ca). We can arrange appointments during our business hours of 8:30am-4:30pm, Monday through Friday.
What to Bring to Your Appointment
When you initially contact our office to set up your appointment, one of our caseworkers will let you know if there are any specific things you should bring with you based on the information you have provided us about your issue. However, to help us serve you better, it is always best to bring along any documentation you have that is related to your issue. For example, if you are facing a barrier with a tax problem, you should bring your tax returns, Notices of Assessment, SIN card, etc. Please also bring a piece of ID.
*If you require assistance with the Canada Revenue Agency (CRA), you must fill out this form authorizing your Member of Parliament’s office to speak to the CRA on your behalf. The CRA cannot speak to us about your case without your written authorization. Please download and print the form and bring it to your appointment.
How to Get Here
The Community Office is located at 885 Adelaide Street North, London ON, on the Southwest corner of Adelaide Street and Grosvenor Street. Guest main entrance is on Grosvenor Street (with the Canada flag). Accessibility-friendly entrance is off our parking lot at the top of the ramp (for individuals with mobility issues only).
By Bus: The Numbers 16 and 92 buses run right by our office on Adelaide Street North. The Number 21 runs along Cheapside Street (a 3 minute walk South to our office), and the Numbers 4 and 17 run along Oxford Street (a 6 minute walk North to our office). We suggest you consult the London Transit Commission’s trip planner to determine the best route from your home to our office.
Parking: We have free parking on-site in the gravel lot beside our office (off Grosvenor). There is also ample free street parking on Grosvenor and the surrounding streets.
